Pathology Decision Matrix

Study the condition, then practice the decision: postpone, avoid locally, modify, or gather more information. Actual decisions depend on current presentation, not the diagnosis label alone.

Musculoskeletal

ConditionCore conceptMassage reasoning
Acute fractureBreak in bone continuityDo not stress the injured region; follow medical restrictions/healing status.
OsteoporosisReduced bone strengthModify pressure/leverage/positioning; fracture risk matters.
OsteoarthritisDegenerative whole-joint diseaseWork to tolerance; avoid forcing painful/irritable joints.
Rheumatoid arthritisSystemic autoimmune inflammatory arthritisAvoid aggressive work over acute inflammatory flare; consider systemic symptoms/medications.
BursitisIrritated/inflamed bursaAvoid direct aggressive compression of acute bursa.
SprainLigament injuryAcute swelling/instability needs protection and possibly evaluation.
StrainMuscle/tendon injuryAcute severity and healing phase determine local work.
TendinopathyLoad-related tendon disorderAvoid provocative aggressive friction on highly irritable tissue.
Adhesive capsulitisPainful shoulder stiffness/capsular restrictionComfortable work/ROM only; forcing motion is not the goal.
Gout flareUrate-crystal inflammatory arthritisHot, acutely painful joint = local avoidance.
Joint replacementProsthetic joint after surgeryConsider healing stage, precautions, scar status and provider restrictions.

Nervous

Peripheral neuropathyPeripheral nerve dysfunctionModify pressure/temperature because sensation may be unreliable.
Carpal tunnel syndromeMedian nerve compression at wristAvoid provoking neural symptoms; do not promise to “release the nerve.”
RadiculopathyNerve-root dysfunctionRadiating symptoms/weakness require careful screening; progressive deficit warrants referral.
Multiple sclerosisImmune-mediated CNS demyelinationAdapt to fatigue, heat sensitivity, mobility and current symptoms.
Parkinson diseaseProgressive movement disorderConsider mobility, rigidity, balance, fatigue, medication timing and transfer safety.
Stroke historyPrior cerebrovascular injuryConsider sensation, weakness, spasticity, communication, medications and transfer safety; acute stroke signs are emergency.

Cardiovascular / blood

Suspected DVTDeep venous clotNo massage; prompt medical evaluation.
Varicose veinsDilated superficial veins/valve dysfunctionAvoid aggressive direct pressure over affected veins.
HypertensionPersistently elevated arterial pressureConsider control, symptoms, meds and orthostatic dizziness. Avoid simplistic universal cutoff rules.
AnticoagulationReduced clotting tendency from medicationBruising/bleeding risk may require pressure modification.
AnemiaReduced oxygen-carrying capacity/hemoglobin depending on typeFatigue, exertional tolerance and underlying cause matter.

Integumentary / infectious

PsoriasisImmune-mediated inflammatory skin diseaseNot contagious; modify around cracked, painful or irritated plaques.
TineaFungal infectionAvoid affected contagious area; prevent cross-contamination.
Herpes active lesionViral lesionLocal avoidance and hygiene precautions.
CellulitisBacterial skin/subcutaneous infectionDo not massage infected region; medical treatment needed.
Open woundLoss of skin integrityLocal avoidance; contamination/infection-control precautions.

Endocrine / metabolic

DiabetesGlucose-regulation disorderScreen neuropathy, wounds, vascular status and glucose/medication timing.
HypoglycemiaLow blood glucoseRecognize sweating, shaking, hunger, confusion; severe symptoms require emergency response.
Hyperthyroid stateExcess thyroid-hormone effectConsider heat intolerance, tachycardia, anxiety and medical stability.
Hypothyroid stateLow thyroid-hormone effectConsider fatigue, cold intolerance and associated symptoms; do not massage thyroid to alter hormones.

Respiratory

AsthmaInflammatory airway disease with episodic narrowingPosition comfortably; acute severe breathing difficulty is emergency.
Acute contagious respiratory infectionTransmission/systemic illness concernPostpone when illness/transmission risk makes treatment inappropriate.
COPDChronic airflow limitationPosition for breathing, pace transitions, monitor tolerance; do not make respiratory cure claims.

Lymphatic / immune

LymphedemaImpaired lymph transportRequires condition-specific management; routine deep massage is not manual lymph drainage.
LymphangitisInflammation/infection of lymphatic vesselsAcute infection needs medical treatment, not massage over region.
LymphomaLymphoid malignancyTreatment planning depends on status, symptoms, oncology care and practitioner competence.
HIVVirus affecting immune systemNot spread by ordinary intact-skin massage; use standard precautions with everyone.

Decision shortcut

Systemic acute illness or embolic/emergency concern → postpone/emergency response. Local contagious/open/acute inflammatory problem → often local avoidance. Stable chronic condition → often modification based on symptoms and complications. Unclear risk → gather more information rather than guessing.
