Intake identifies goals, preferences, risks, relevant history and consent needs. It is not paperwork performed merely for legal protection.
Ask about current symptoms and reason for visit, diagnoses, injuries/surgeries, medications, allergies/sensitivities, pregnancy status when relevant, skin conditions, cardiovascular/neurological concerns, recent illness, implanted devices, prior massage responses and healthcare restrictions/recommendations.
Ask what the medication is for and what effects matter to massage safety. Anticoagulants can increase bruising/bleeding risk; analgesics can alter pain feedback; antihypertensives may contribute to dizziness; diabetes medications can affect glucose. Do not instruct clients to stop prescribed medication.
A form cannot replace conversation. Clarify ambiguous answers, ask about changes since the last visit and verify current symptoms. Before each session, a brief update can catch new injuries, illness, medication changes or procedures.
Collect information relevant to care, protect records, discuss sensitive information discreetly and avoid unnecessary disclosure. Explain why sensitive questions are being asked.
Explain the proposed work, relevant areas, draping/positioning and meaningful alternatives. Consent is ongoing and can be changed or withdrawn at any time.
